Fragility and the Risk Paradox
Can well-meaning safety strategies actually end up increase risk? Can some safety initiatives actually make systems and processes more prone to failure (more fragile)?
In this presentation we will explore the concept of fragility (as it relates to organisations and systems), how it interacts with risk and safety, and how an understanding of it can help us see why some traditional safety strategies and initiatives can actually end up increasing risk (the Fragility and Risk Paradox).
We will also touch on some foundational strategies and concepts that can be applied within any organisation to help mitigate (the often inevitable) introduction of fragility over time.
